Objective: To determine in situ serum cortisol levels in patients undergoing third molar surgery. The sample consisted of 30 patients, from whom blood samples were taken at three different times (pre-, intra-, and post-operative) to determine serum cortisol levels using the ELISA technique. The values were transferred to a database and processed using descriptive statistical analysis (mean and standard deviation) and inferential analysis (Kruskal-Wallis and Tukey's tests). Results: Preoperatively, 66.7% (n = 20) of patients had serum cortisol levels within the normal range (50-230 ng/mL), while 33.3% (n = 10) had levels above 230 ng/mL. Intraoperatively, 60% (n = 18) of patients presented cortisol levels higher than 230 ng/mL, and postoperatively, this was observed in 56.7% (n = 17). A statistically significant increase in serum cortisol was found intraoperatively (285.53 ± 108.00 ng/mL) compared to preoperative values (198.00 ± 74.85 ng/mL), and this elevated level was sustained in the postoperative period (280.67 ± 113.09 ng/mL), although no statistically significant difference was observed between intra- and postoperative values. Conclusion: The present work reports significant in situ increases in serum cortisol levels due to the physiological alteration caused by surgical trauma, mantained until the post-operative period. |

Materiales y métodos: La población estuvo conformada por pacientes del Hospital Universitario-Docente de Chimbote, los cuales pasaron por criterios de inclusión y exclusión para su selección. La muestra constó de 30 pacientes, a quienes se les realizaron tomas de muestras de sangre en tres momentos (pre, intra y postoperatorio), para después analizarlas mediante la técnica de ELISA y determinar los niveles séricos de cortisol. Los valores fueron transferidos a una base de datos y procesados mediante análisis estadísticos descriptivos (media y desviación estándar) e inferenciales (Kruskal-Wallis y test de Tukey). Resultados: En el preoperatorio, se encontró que el 66,7 % (n = 20) de los pacientes presentaron niveles séricos de cortisol con valores normales (50-230 ng/mL), y el 33,3 % (n = 10) tuvo niveles mayores a 230 ng/mL En el intraoperatorio, el 60,0 % (n = 18) presentó niveles séricos de cortisol mayores a 230 ng/mL; y en el postoperatorio, el 56,7 % (n = 17) presentó niveles séricos de cortisol mayores a 230 ng/mL. Asimismo, se reportó un incremento estadísticamente significativo de los niveles séricos de cortisol en el momento intraoperatorio (285,53 ± 108,00 ng/mL) respecto al preoperatorio (198,00 ± 74,85 ng/mL), el cual se mantuvo hasta el postoperatorio (280,67 ± 113,09 ng/mL) sin presentar diferencias estadísticamente significativas. Conclusión: Se reportaron aumentos significativos en los niveles séricos de cortisol in situ debido a la alteración fisiológica asociada al trauma quirúrgico, los cuales se mantuvieron hasta el postoperatorio.Objetivo: Determinar os níveis séricos de cortisol no sangue in situ em pacientes submetidos à cirurgia de terceiros molares. A amostra consistiu em 30 pacientes, que foram submetidos à coleta de sangue em três momentos (pré, intra e pós-operatório), que foram então analisados usando a técnica ELISA para determinar os níveis de cortisol sérico. Os valores foram transferidos para um banco de dados e processados por análises estatísticas descritivas (média e desvio padrão) e inferenciais (Kruskal-Wallis e teste de Tukey). Resultados: No pré-operatório, 66,7% (n = 20) dos pacientes apresentavam níveis normais de cortisol sérico (50-230 ng/mL) e 33,3% (n = 10) tinham níveis superiores a 230 ng/mL. No intraoperatório, 60,0% (n = 18) apresentaram níveis de cortisol sérico superiores a 230 ng/mL; e no pós-operatório, 56,7% (n = 17) apresentaram níveis de cortisol sérico superiores a 230 ng/mL. Foi registrado um aumento estatisticamente significativo nos níveis de cortisol sérico no intraoperatório (285,53 ± 108,00 ng/mL) em comparação com o pré-operatório (198,00 ± 74,85 ng/mL), que foi mantido até o pós-operatório (280,67 ± 113,09 ng/mL) sem diferenças estatisticamente significativas.
