The sample was selected from analysis of 2986 medical records from patients treated between 1999 and 2008 by the Department of Oral and Maxillofacial Surgery of the Faculty of Dentistry of Piracicaba, State University of Campinas (FOP-UNICAMP), State of São Paulo, Brazil. During this period, 757 patients under 18 years were treated, of which 220 were victims of bicycle accidents. The bicycle accidents constituted 29 % of the facial trauma. Only 1,8 % of the patients made use of safety devices. The adolescents were the most affected and the male gender was the most prevalent. 69 facial fractures were identified, being the jaw the most affected (55,1 %). The conservative treatment was used in most of the cases (72,4 %), while only one case of postoperative complication was reported. It is concluded that bicycle accidents represented the main etiology of the maxillofacial trauma, being the male gender predominant in all age groups evaluated. The mandibular fractures were the most prevalent and the majority of patients did not use any type of safety device.El objetivo fue realizar un análisis retrospectivo de la incidencia, características, uso de dispositivos de seguridad, tratamiento y complicaciones de los traumatismos maxilofaciales causados por accidentes ciclísticos. La muestra fue seleccionada a partir del análisis de 2 986 historias clínicas de pacientes atendidos entre 1999 y 2008 por el Área de Cirugia Buco-Maxilo-Facial de la FOP-UNICAMP, Estado de São Paulo-Brasil. Durante este periodo, 757 pacientes menores de 18 años fueron atendidos. De éstos, 220 fueron víctimas de accidentes ciclísticos. Los accidentes ciclísticos representaron el 29 % del trauma facial. Solo el 1,8 % de los pacientes hicieron uso de dispositivos de seguridad. Los adolescentes fueron los más afectados, y el género masculino el de mayor prevalencia. Fueron identificadas 69 fracturas faciales, siendo la mandíbula la más afectada (55,1 %). El tratamiento conservador fue utilizado en la mayoría de los casos (72,4 %), mientras que solamente un caso de complicación pos-operatoria fue reportado. Se concluye que los accidentes ciclísticos representaron la principal etiología del trauma maxilofacial, siendo el género masculino predominante en todos los grupos etarios evaluados.
