With the aim of presenting a case from the radiological point of view of sacralization of L5 associated with low back pain, we present a patient who presents low back pain for 3 months; it is exacerbated with flexor-extensor and rotational movements of the trunk, he suffers a fall from his own height so an anteroposterior (AP) study of the pelvis and lateral sacro-coccyx is requested where findings related to Bertolotti's Syndrome are observed. This syndrome should be considered in the differential diagnosis of patients with low back pain with or without radiculopathy. Keys words: Low Back Pain; Bertolotti´s syndrome; Technology, Radiologic.  El síndrome de Bertolotti se lo define como lumbalgia con o sin dolor radicular, asociado a anomalía congénita en la morfología vertebral (megapófisis transversas con o sin fusión con alas de sacro o crestas iliacas), consecuencia de ello se observa cambios osteodegenerativos facetarios, discales en niveles superiores a la vértebra de transición, aunque también puede manifestarse al mismo nivel con patología discal compresiva extraforaminal. Con el objetivo de exponer de un caso desde el punto de vista radiológico de sacralización de L5 asociado a dolor lumbar, se presenta un paciente que presenta lumbalgia por 3 meses; se exacerba con movimientos flexo extensores y rotacionales del tronco, sufre caída desde su propia altura por lo que se solicita estudio anteroposterior (AP) de pelvis y lateral de sacro-coxis donde se observa hallazgos en relación con Síndrome de Bertolotti. Este síndrome debe considerarse en el diagnóstico diferencial de pacientes con lumbalgia con o sin radiculopatía.
