This would be the first case reported in our country.Se presenta el caso de una mujer de 39 años con enfermedad de Graves y con episodios de hipoglucemia sintomática persistente, con valores elevados de péptido C e insulina. El estudio de imágenes no mostró la presencia de tumores. Los anticuerpos antiinsulina fueron positivos y el estudio genético con tipificación del HLA clase II expresó los alelos DRB1*04 y DQB1*03*02 relacionados a enfermedad de Hirata.
