A comprehensive search was conducted across multiple databases, including PubMed, Scopus,Embase, and Google Scholar, identifying five cohort studies with robust methodological quality. These studies includedbetween 58 and 207 premature infants, most of whom were born at less than 28 weeks of gestational age. Although someindividual findings suggested that higher levels of ARA or DHA might have a protective effect, the results of the meta-analysisdid not demonstrate a statistically significant association between these fatty acids and the development or severity ofROP. The pooled odds ratio (OR) was 0.96 for ARA and 0.71 for DHA, with no significant heterogeneity among the studies.In conclusion, current evidence does not support the hypothesis that plasma levels of ARA and DHA predict the severityof ROP. Further research with larger sample sizes and more homogeneous methodologies is required to provide morerobust evidence.La revisión sistemática y metaanálisis analiza si los niveles plasmáticos de ácido araquidónico (ARA) y ácido docosahexaenoico (DHA) se asocian con la severidad de la retinopatía de la prematuridad, una causa importante de ceguera en recién nacidos prematuros. Para ello, se buscaron estudios observacionales en PubMed, Scopus, Embase y Google Scholar, y se seleccionaron cinco investigaciones de cohorte con buena calidad metodológica. Los estudios incluyeron entre 58 y 207 bebés prematuros, en su mayoría con menos de 28 semanas de edad gestacional. Aunque algunos hallazgos individuales sugirieron que niveles más altos de ARA o DHA podrían tener un efecto protector, los resultados combinados del metaanálisis no mostraron una asociación estadísticamente significativa entre estos ácidos grasos y la aparición o severidad de la enfermedad. En el caso del ARA, el OR combinado fue 0.96, y para el DHA fue 0.71, sin heterogeneidad entre los estudios analizados.
