Disseminated hydatidosis is the simultaneous involvement of multiple organs and, as rare, is a diagnostic challenge. We present the case of a man with disseminated hydatidosis complicated by the rupture of one of the lung cysts, which had been incidentally diagnosed by imaging studies. Due to pulmonary, hepatic, pancreatic and splenic involvement, he received albendazole for 9 uninterrupted months with the disappearance of most of the lesions at the lung, hepatic and splenic levels. Currently, the management of disseminated hydatidosis there is no consensus, although surgery, percutaneous drainage and medical treatment remain fundamental pillars for hydatidosis. The interest of this publication is the management and follow-up of medical treatment of this rare presentation of disseminated hydatidosis.La hidatidosis es una zoonosis parasitaria ocasionada por el Echinococcus, siendo las zonas rurales ganaderas las de mayor prevalencia. Constituye un problema de salud pública, siendo el Perú uno de los países más afectados. El género granulosus es el responsable de la enfermedad quística, afectando predominantemente al hígado y pulmones. La hidatidosis diseminada es la afectación simultánea de múltiples órganos y, por ser infrecuente, la convierte en un reto diagnóstico. Presentamos el caso de un varón con hidatidosis diseminada complicada con la ruptura de uno de los quistes pulmonares, habiéndose diagnosticado incidentalmente a través de estudios de imagen. Debido al compromiso multisistémico recibió albendazol durante 9 meses ininterrumpidos evidenciándose la desaparición de la mayoría de las lesiones pulmonares, hepática y esplénica. Actualmente, en el manejo de la hidatidosis diseminada no hay consenso, aunque la cirugía, el drenaje percutáneo y el tratamiento médico, siguen siendo pilares fundamentales en la hidatidosis.
