Studies reporting maternal mortality due to COVID-19 were included. In addition, the results were compared with epidemiological reports proposed by the Peruvian Ministry of Health (MINSA). Results: Six studies with 44 pregnant women who died from COVID-19 were included. The studies covered four regions of Peru (Lima, Cajamarca, Huancayo, and Callao). Significant maternal complications were observed, including pre-eclampsia, eclampsia, prematurity, intrauterine growth restriction, and fetal death. Among the most frequent comorbidities were hypertension, diabetes, and obesity. According to MINSA epidemiological data, a total of 1,486 pregnant women were reported to have died between 2020 and 2023. Of these, 15.1% (n = 224) of deaths were directly attributed to COVID-19. Conclusions: COVID-19 represents a significant risk for pregnant women in Peru, with deaths attributed to the disease and severe maternal complications such as pre-eclampsia and prematurity, highlighting the need for maternal health care during the pandemic. It is essential that health authorities protect pregnant women from COVID-19 through vaccination, education on preventive measures, and specialized monitoring, especially for those with comorbidities.

Material y métodos: Después del registro del protocolo de investigación en PROSPERO (CRD42024506108), se realizó una revisión de los estudios durante el período comprendido entre el enero del 2020 a diciembre 2023 en las bases de datos PubMed, Scopus, Ovid, Web of Science, Google Académico, Biblioteca Virtual de Salud y Scielo mediante una búsqueda avanzada y el empleo de términos como ['COVID-19' AND ('embarazo' O 'gestante') AND Perú]. Se incluyeron estudios que informaron la mortalidad materna por COVID-19. Además, los resultados se compararon con los informes epidemiológicos propuestos por el Ministerio de Salud de Perú (MINSA). Resultados: Se incluyeron 6 estudios con 44 gestantes fallecidas por causa de la COVID-19. Los estudios abarcaron cuatro regiones del Perú (Lima, Cajamarca, Huancayo y Callao). Se observaron complicaciones maternas significativas, como preeclampsia, eclampsia, prematuridad, restricción del crecimiento intrauterino y óbito fetal. Entre las comorbilidades más frecuentes se encontraron la hipertensión, diabetes y obesidad. Según los datos epidemiológicos del MINSA, se reportó un total de 1,486 mujeres embarazadas fallecidas en el período comprendido entre 2020 y 2023. De estas, el 15.1% (n=224) de los decesos se atribuyó directamente a la COVID-19. Conclusiones:  La COVID-19 representa un riesgo significativo para mujeres embarazadas en Perú, con fallecimientos atribuidos a la enfermedad y complicaciones maternas graves como preeclampsia y prematuridad, destacando la necesidad de atención a la salud materna durante la pandemia.
