Hospital Nacional del Sur-EsSalud-Arequipa (1994-1997)La inestabilidad aguda de la rodilla se debe fundamentalmente a la ruptura del ligamento cruzado anterior (LCA), la que es provocada por traumatismos externos, desarrollados en nuestro cotidiano vivir. El crecimiento demográfico, industrializaci6n, incremento del parque automotor y el deporte competitivo son determinantes en la incidencia de las rupturas ligamentarias. El ligamento cruzado anterior (LCA), tiene una anatomía, vascularidad e inervación sui generis; el cual, siendo intrarticular es extrasinovial, se nutre de plexos vasculares periligamentarios y por absorción, y transmite impulsos nerviosos propioceptivos. Cuando se rompe, cicatriza de una manera an6mala y nunca recupera su normalidad anatómica y funcional. Existe la tendencia de tratar esta patología con técnicas quirúrgicas de sutura de los extremos proximal y distal del ligamento cruzado anterior roto, o sutura más aumentación -más comúnmente conocido como refuerzo-, utilizando estructuras anat6micas como el tendón del semitendinoso, que permitían recuperar parcialmente la funcionalidad de la rodilla. Esto ha determinado que se aplique una tercera técnica quirúrgica denominada reconstrucci6n del ligamento cruzado anterior con autoinjerto hueso-tend6n rotuliano-hueso, técnica que sólo estaba indicada para la inestabilidad cr6nica de la rodilla. Nuestra investigaci6n se ha dedicado a comparar estas tres técnicas quirúrgicas empleando para ello las escalas internacionales de Funcionalidad de Lysholm y de Estabilidad de Marshall. Demostrando categ6ricamente y con pruebas estadísticas de alta significación, que la técnica de reconstrucción hueso-tendón rotuliano-hueso es la mejor, proporcionando de buena a excelente funcionalidad y estabilidad de la rodilla postquirúrgica en rupturas agudas del ligamento cruzado anterior (LCA). Se tiene una variable determinante en la evolución, que es la edad, dado que los pacientes j6venes evolucionan mejor que los ancianos. El autoinjerto hueso-tend6n rotuliano-hueso se osteointegra adecuadamente, y la propiocepci6n se va reinstalando progresivamente. Mecanismos que permiten que los pacientes se reintegren a sus actividades laborales y muchos de ellos puedan llegar, incluso, a practicar deporte.Acute instability of the knee is owed fundamentally to the rupture of the anterior crossed ligament (ACL), caused by externa! traumatisms, developed in our daily one to live. The demographic growth, industrialization, increment of the self-motor park and the competitive sport are decisive in the incidence of the ligament ruptures. ACL has an anatomy, vascularity and innervation Sui generis; being intrarticular is extrasynovial, it is nurtured of periligamentous vascular plexus and for absorption, and it transmits proprioceptives nervous impulses. When it breaks, it heals in an anomalous way and it never recovers their anatomical and functional normality. Tendency towards this pathology treatment is with surgical technical of suture the proximal and distal extremes of broken anterior crossed ligament, or augment suture -commonly well-known as reinforcement-, using anatomical structures as the tendon of the semitendinous that allowed to recover partially knee functionality. This has determined that a third technique surgical denominated reconstruction of the anterior crossed ligament with autograft bone-patellar tendon-bone, technique is applied that was only suitable for the chronic instability of the knee. Our investigation has been devoted to compare these three surgical techniques, using the internationals Lysholm's Functionality scale, and Marshall's. Stability scale. Demonstrating categorically and with statistical high significance tests that the technique of reconstruction bone-patellar tendon bone is best, providing of good to excellent functionality and stability at postsurgical knee in sharp ruptures of the anterior crossed ligament (ACL). A decisive variable in the evolution that is the age, since the young patients evolve better than the old men. The autograft bone-patellar tendon-bone is appropriately osteointegrated, and the proprioception leaves reinstalling progressively.
