Sumario:Results of a longitudinal prospective study done between 1988 and 1993 to determine the predictive  value in fetal wellbeing of the ultrasound biophysical profile (UBP) created by the first author a represented. The UBP considers seven feto-placental parameters: fetal movements, respiratory movements, fetal tone, basal heart frequency, cardiac reactivity, amniotic fluid volume and placental maturity. We assigned two points to each normal parameter. We performed 1434 studies in high risk pregnancies over 27 week; with presumptive diagnosis of prolonged pregnancy, pregnancy induced hypertension, fetal distress, intrauterine fetal growth retardation, premature rupture of membranes and others. We considered normal air UBP score of 11 to 14, and correlated score; with one or more of the following adverse perinatal results: clinical oligohydramnios, thick meconial amniotic fluid, Apgar less than 8 at 5 minutes, neonatal convulsions perinatal death. UBP value were: sensitivity 82,6%, specificity 98,8%, positive predictive value 88,8%, negative predictive value 98,1% false positive index 11,2% false negative index 1,9%; prevalence 10%. Cesarean-section rate in the study group was lass than expected and the perinatal mortality 0,69 x 1000 l.b. was much less than The global hospital rate. UBP appear as an important tool in fetal surveillance.
