Objetivo general: Determinar la relación entre la deficiencia de vitamina B12 y depresión unipolar en el adulto mayor atendido en consulta externa de psiquiatría del Hospital María Auxiliadora, 2017. La deficiencia de Vitamina B12 o Cobalamina, se ha considerado usualmente una deficiencia nutricional asociada a anemia megaloblástica; sin embargo, también es un factor presente en la etiopatogenia de múltiples patologías; una de ellas, y la menos estudiada, es la depresión unipolar en el paciente adulto mayor. La Cobalamina o B12 es esencial para sintetizar ácidos grasos, mielina y aminas presentes en el sistema nervioso central (SNC), y por ende debemos considerar un papel importante en la producción de neurotrasmisores, los cuales disminuyen en la depresión, así como en la misma estructura neuronal. The case of a 21-year-old man, hospitalized in the psychiatric service of a general hospital with a history of abnormal thought, perception, and behavioral processes, reaching a suicidal attempt, is presented. In the psychiatric hospitalization unit, he experienced a torpid clinical course, including the appearance of involuntary movements and a progressive sensory disorder that led to his transfer to the Intermediate Care Unit of the Internal Medicine Service. After laboratory tests, imaging support and evaluation by different medical specialties, the final diagnosis was a clinical picture compatible with NMDA anti-receptor encephalitis.

Among the so-called delusional disorders, the erotomanic subtype is characterized by, the delusional belief that a person, usually of higher social and intellectual status, is in love with the patient. The case of a young man whose clinical picture consists in his romantic relationship with an Argentinian singer who fell in love with him through the screen and communicates by telepathic connections, is presented. Erotomanic delirium is an uncommon and poorly documented pathology, particularly in mental health areas of general hospitals. It is important to remain awareness of cases like this in order to provide reliable information and make it possible a timely diagnosis and treatment.
