Informar un caso de Gangrena de Fournier (GF) causado por una apendicitis aguda retrocecal perforada, que causo daño en el escroto y testículo, suceso indiscutiblemente excepcional. Atendido en el Servicio de Cirugía General del Hospital de Alta Complejidad “Virgen de la Puerta” (HACVP). Material y métodos: Realizaremos la descripción del caso de un paciente que durante su ingreso presenta un cuadro clínico de dolor abdominal tipo cólico en región subcostal derecha asociado a un cuadro séptico generalizado. Resultados: La GF cuyo origen principalmente deriva de patologías anorrectales y urológicas, no pudiéndose descartar en algunos casos un origen intraabdominal.
