A 24-year-old woman was admitted with seizures and severe hyponatremia associated with episodes of tachycardia and sustained arterial hypertension. During her hospitalization she presented muscle weakness and altered sensibility of upper limbs, progressing to all extremities with signs of acute denervation by electromyography. With suspicion of Guilláin-Barré syndrome, therapy with intravenous immunoglobulin was started. A history of previous episodes with recurrent abdominal pain, associated with the current picture of hypertension, tachycardia and seizures led us to consider acute intermittent porphyria as a differential diagnosis. The elevated values of delta amino levulinic acid in blood and porphobilinogen in urine confirmed the diagnosis. Background: Hospitals should evaluate their antibiotics use and the relationship with local bacterial resistance. Objective: Identify the trend in the consumption of carbapenems and explore its correlation with multiresistant Pseudomona aeruginosa cultures. The study: An ecological study was carried out in a hospital in Lima – Peru. We measured the consumption of imipenem and meropenem in one year and calculated the correlation with multiresistant Pseudomona aeruginosa cultures. Findings: The consumption of imipenem had no significant variations, but meropenem had a tendency to decrease. There was a significant positive correlation between the defined daily dose of meropenem according to hospital stay and discharges with multiresistant Pseudomona aeruginosa cultures (0.71, p=<0.01 and 0.64, p=0.02, respectively). Se realizó un estudio ecológico para identificar la tendencia en el consumo de imipenem y meropenem en el Servicio de Medicina Interna del Hospital Guillermo Kaelin de la Fuente durante el año 2016 y conocer su correlación con la frecuencia de cultivos positivos para Pseudomona aeruginosa multirresistente. Para ello se midió el consumo de los antimicrobianos con la dosis diaria definida (DDD) según estancias y según egresos hospitalarios, resultando que el consumo de imipenem se mantuvo similar a lo largo del año a excepción de un incremento temporal hacia la mitad del periodo de estudio. En el consumo de meropenem por el contrario si hubo una tendencia a la disminución, tanto para la dosis diaria definida según estancias como la dosis diaria definida según egresos.
