Retroalimentación es el acto de comunicar al alumno información sobre su desempeño en la práctica clínica con el fin de modificar su conducta para lograr un aprendizaje. La realización de esta actividad requiere de la capacitación del docente y la preparación de los alumnos, así como la programación a lo largo de los cursos y la inversión de tiempo y esfuerzo. Existe evidencia de los numerosos beneficios de la retroalimentación. Se presenta la experiencia del uso de la retroalimentación en un curso de pregrado de una facultad de medicina en nuestro país.

INTRODUCTION: The educational environment is directly related to the learning and well-being of residents and patient care. Assessment of the educational environment is a necessity of medical residency programs. There are no validated instruments in our country to evaluate educational environment in medical residency. OBJECTIVE: To evaluate the validity and reliability of the PHEEM questionnaire for measuring educational environment in a medical residency program. METHODOLOGY: A cross-cutting study with a qualitative and quantitative approach was conducted. Cultural adaptation, content validity and construct validity were evaluated. Medical residency is a period of intense learning in service. New trends in medical education also involve medical residents and we are currently witnessing its evolution. These new trends involve changes in the organization of programmes and in the design of competency-based specialization programs, innovation in teaching strategies and evaluation strategies, the care of the learning climate and the accreditation of programs. A summary of these trends is presented and some national and international experiences are described.

La comunicación de malas noticias es una habilidad clínica que todo médico debe poseer. En este estudio buscamos reconocer la manera en que los médicos residentes en un hospital nacional realizan la comunicación de malas noticias. Se encuestó a 19 residentes en un hospital público en el 2015. Se obtuvo que 100 % de los residentes no habían recibido ninguna capacitación sobre el tema a pesar que todos ellos habían dado alguna mala noticia en el último mes; 68,4 % reportaron tener sentimientos de frustración al dar malas noticias; 53,3 % de residentes reportaron que no había un lugar adecuado para dar malas noticias. Se concluye que la comunicación de malas noticias es una actividad frecuente en la práctica médica pero que generalmente no se enseña.

Breaking bad news is a clinical skill that every physician should possess. In this study, we seek the way in which the medical residents were communicating bad news in the hospital. Nineteen residents were surveyed in a public hospital in 2015. It was found that 100 % of residents had not received any training on the subject although all had given some bad news in the last month. Feelings of frustration in breaking bad news were found in 68,4 %. Besides, 53,3 % of the residents reported that there was not an adequate place to give bad news. It is concluded that breaking bad news is a frequent activity in medical professional practice but it is not generally taught.
