Controversies in the treatment of incomplete abortion, MVA versus medical treatment with misoprostol

Non-complicated incomplete abortion is one of the most frequent obstetric emergencies in our health facilities, causing high care costs. There are two current modern methods for its attention: manual vacuum aspiration (MVA) and medical treatment with misoprostol, both with the possibility of outpatient management, reducing costs to the health system. Controversies between the two technologies are analyzed and it is concluded that both MVA and misoprostol treatments are modern technologies applicable in gynecology and obstetrics services in our country, with same safety and effectiveness by evidence of many studies conducted.

Man reproduces sexually, by sexual intercourse, and there is the possibility to transmit a series of diseases by this route. Pelvic inflammatory disease (PID) involves upper reproductive tract inflammatory disorders. We review in this article PID pathogenesis, immunologic factors, immunity against bacteriae, adaptation mechanisms, all these concepts necessary for treatment based on knowledge of PID causative agents and host response.

The obstetrician and gynecologist must frequently face complicated situations in the practice of obstetrics, one of them is the rupture of the membranes in a preterm pregnancy. While the medical conduct is to preserve both maternal and fetal health, many times the decision may affect fetal health in order to preserve the maternal health. This occurs when the presence of an infection makes it impossible to maintain the normal course of pregnancy and complete gestational age. In many cases the complications that cause prematurity of the newborn will put mother’s life at risk and we must therefore be prepared to minimize the consequences of our decision. In this article we review evidence-based management that the obstetriciangynecologist should consider when this situation occurs.

High maternal morbidity and mortality, which occur mainly in poor, developing countries, constitute a problem which is very representative of a country’s health system, and are therefore considered as leading health indicators. For this reason, although maternal mortality expressed in absolute values does not represent an exorbitant quantity for a country, every case represents a failure of the country’s health system. Cases of maternal morbidity are more numerous, which is why they could be more useful to enable the authorities to take more effective action to combat maternal death. This article analyzes the publications made in relation to investigations, reports and cases of maternal morbidity and mortality since 1950, that is, for the past 67 years.

We analyze In this presentation there is no big difference in the efficiency and effectiveness between the long-acting reversible contraceptive methods (implants, IUD) known as the LARC methods (by its abbreviation in English) and the definitive long-acting surgical contraception (vasectomy, tubal ligation). The difference between both contraceptive methodologies is related with the temporality and reversibility of one method and the side effects that might originate from this. The low demand of LARC is probably due to the myths and barriers existing among some health providers and the population. Also to the failure in contraceptive advice, limited availability, or high costs for some vulnerable populations like the adolescents. A través de este indicador podemos conocer el esfuerzo que los países hacen para proteger la salud de las mujeres, ya que también la muerte materna es considerada como una expresión de la inequidad de género.
