Objetivo: Determinar los factores de riesgo de rotura prematura de membranas en adolescentes atendidas en el Instituto Nacional Materno Perinatal en enero–diciembre 2015. Metodología: Estudio no experimental, analítico, retrospectivo, transversal. La muestra total estuvo conformada por 164 gestantes adolescentes, divididas en grupo casos (82) y grupo control (82). Para determinar los factores de riesgo, se utilizó la prueba de Chi cuadrado y el cálculo del Odds Ratio. Resultados: La edad promedio de las gestantes fue de 14.5 años. Respecto al nivel educativo, el 59.8% de las gestantes solo alcanzaron el nivel primario y el 40.2%, el nivel secundario. El 90.9% de las gestantes adolescentes eran primigestas y el 65.9% tuvieron un control prenatal insuficiente. Introduction: Uterine rupture in the context of placenta accreta spectrum poses a significant clinical challenge with critical implications for maternal and fetal health. Early diagnosis and timely intervention in patients with risk factors, such as a history of cesarean deliveries and placenta previa, are essential to prevent severe complications. Case presentation: This report describes the case of a 36-year-old woman with a history of two cesarean sections, who presented at 30 weeks of gestation with acute abdominal pain. Evaluation revealed placenta previa with placental invasion and hemoperitoneum. During exploratory laparotomy, uterine rupture was confirmed, leading to a cesarean delivery and subsequent hysterectomy, resulting in a favorable maternal outcome and neonatal survival.
