Introduction: Congenital anomalies and acquired diseases of the uterus have been obstacles for successful treatment of infertility. The uterine cavity is where the embryo normally implants. It is important a proper evaluation of the uterine cavity to rule out or detect changes that may affect implantation. Hysterosonography is a diagnostic method used in the evaluation of the uterine cavity. The most common indication for sonohysterography in patients with infertility is suspected intrauterine pathology. Objectives: To describe routine sonohysterography findings in an infertile population, with or without suspicion of endometrial pathology, in relation to patient’s age. Objective: To determine the diagnostic sensitivity of transvaginal ultrasonography and sonohysterography compared with hysteroscopy in patients with infertility. Design: Descriptive study. Setting: Procrear Fertility Clinic, Lima, Peru. Participants: Women with infertility. Methods: Between January and December 2 013 patients studied for infertility underwent surgical hysteroscopy; corresponding transvaginal ultrasound and hysterosonography prior to surgery were analyzed for most frequent diagnoses and sensitivity of the studies. Analysis was done using SPSS 15 software. Main outcome measures: Sensitivity of hysteroscopy, transvaginal ultrasound, and hysterosonography. The case of an infertile 22 year-old woman with 3 years of cohabitation and history of menarche and premature ovarian failure (POF) at age 15 is presented. Secondary sex characteristics were normal, FSH was 69 mIU/mL, LH 21.5 mIU/mL, estradiol 5 pg/mL, normal thyroid profile, prolactin 0.22 ng/mL, glucose and insulin within normal limits; no alterations of the pituitary gland were detected by magnetic resonance. Karyotype was 46, XX. Transvaginal ultrasound showed a 46 x 20 x 25 mm hypothrophic uterus, endometrium 2.2 mm, hypotrophic ovaries. Administration of estradiol valerate/dienogest enlarged uterus to 69 x 22 x 33 mm (in one month) and 72 x 24 x 30 mm (in one year).
