Much has been discussed about why after the International Conference of health of Alma Ata of 1978 has not followed a momentum that made possible its implementation and more health goes by not only different but opposite currents in some cases. While it is true that this discussion is important to better understand the lessons learned around the so-called primary health care, avoid errors and deepen its achievements, it is more important to operate, in the sense that Alma Ata proposed: comprehensive and inclusive health models that reflect justice, equity, innovation and boldness needed to build a better, more just, healthy, developed and happy world. In the Americas there are relatively recent experience advocating for it, that it is important to present initially, since they are still in the process of systematization and research.

Alma Ata is still in force in those experiences of comprehensive health care from all and for all that some countries have managed to implement, especially in the rural areas with populations affected by exclusion and poverty. Peru has several of them that find their roots in experiences previous to the 1978 Conference. It is interesting to confirm that many of these were initially dedicated to the care of pregnant women and newborns, and then they have translated their lessons learned to the care of the whole population.

Objective: The operationalization and the role of a participatory model of diagnostic and therapeutic formulations called Foster Groups, implemented in a Community Mental Health Center in Lima, Peru, are analyzed. Material and Methods: The research was based on the participanting observation by experts in community mental health, public health, communication and social sciences, of a total of 24 Foster Group sessions held at the Nancy Reyes Community Mental Health Center in Chorrillos, Lima Peru, during the years 2018-2019. Results: The Foster Groups are structured in sequential phases that follow a logical process of diagnostic formulations and management plans, concurrently performed by professionals from different disciplines for a small group of users, thus allowing both, diagnoses as plans, to be more comprehensive.
