Se estudió a un total de 114 pacientes adultos que cumplieron con los criterios de selección que fueron atendidos en el Programa Contra la Tuberculosis del Hospital Distrital de Laredo durante enero del 2020 - junio del 2024. RESULTADOS: Se determinó que, de un total de 80 pacientes diagnosticados con tuberculosis pulmonar mediante cultivo, 75 tuvieron pruebas moleculares GeneXpert MTB/RIF positivas y de un total de 34 pacientes descartados de tener tuberculosis pulmonar mediante cultivo ,30 tuvieron pruebas moleculares GeneXpert MTB/RIF negativas. Con estos datos se calculó una sensibilidad del 93.75%, una especificidad del 88.24%, un valor predictivo positivo del 94.94%, un valor predictivo negativo del 85.71%, una razón de verosimilitud positiva de 7.79 y una razón de verosimilitud negativa de 0.07. Adicionalmente, la variable característica del esputo tuvo una asociación estadísticamente positiva con la aparición de falsos positivos (p<0.001). CONCLUSIÓN: La prueba molecular GeneXpert MTB/RIF en pacientes con baciloscopía negativa tuvo adecuada validez diagnóstica.

METHOD: A diagnostic test study was conducted. A total of 114 adult patients who met the selection criteria and were treated in the Tuberculosis Program of the Laredo District Hospital during January 2020 - June 2024 were studied. RESULTS: It was determined that of a total of 80 patients diagnosed with pulmonary tuberculosis by culture, 75 had positive GeneXpert MTB/RIF molecular tests and of a total of 34 patients ruled out for pulmonary tuberculosis by culture, 30 had negative GeneXpert MTB/RIF molecular tests. With these data, a sensitivity of 93.75%, a specificity of 88.24%, a positive predictive value of 94.94%, a negative predictive value of 85.71%, a positive likelihood ratio of 7.97 and a negative likelihood ratio of 0.07 were calculated. Additionally, the characteristic variable of sputum had a statistically positive association with the appearance of false positives (p<0.001).
