Se analizaron variables clínicas, laboratoriales y radiológicas. Se realizó análisis bivariado y regresión logística para determinar factores predictivos de isquemia intestinal. Resultados: Se incluyeron 104 pacientes, de los cuales 35.58% presentaron isquemia intestinal. En el análisis bivariado, los signos peritoneales (p=0.001), el líquido libre (p=0.001) y el engrosamiento de la pared intestinal (p=0.001) fueron significativamente más frecuentes en pacientes con isquemia. En la regresión logística, los signos peritoneales (ORa 28.24, IC 95%: 5.27 – 151.35, p<0.001), el líquido libre (ORa 4.15, IC 95%: 1.12 – 15.29, p=0.033), el engrosamiento de la pared intestinal (ORa 30.67, IC 95%: 6.26 – 150.23, p<0.001) fueron identificados como factores predictivos independientes. El modelo predictivo mostró un AUC de 0.93 (IC 95%: 0.88 - 0.98, p=0.001), indicando una excelente capacidad discriminativa. Conclusiones: Los signos peritoneales, el líquido libre y el engrosamiento de la pared intestinal son predictores significativos de isquemia intestinal en pacientes con obstrucción intestinal por adherencias. Su identificación temprana puede optimizar el manejo clínico y reducir complicaciones

Materials and Methods: We conducted an observational, analytical, cross-sectional study including patients who underwent surgery for adhesive small bowel obstruction at Hospital EsSalud from 2015 to 2024. Clinical, laboratory, and radiological variables were analyzed. Bivariate analysis and logistic regression were performed to determine independent predictors of intestinal ischemia. Results: A total of 104 patients were included, of whom 35.58% had intestinal ischemia. In the bivariate analysis, peritoneal signs (p=0.001), the free liquid (p=0.001), and bowel wall thickening (p=0.001) were significantly more frequent in ischemic patients. In the multivariate logistic regression, peritoneal signs (adjusted OR 28.24, 95% CI: 5.27 – 151.35, p<0.001), the free liquid (adjusted OR 4.15, 95% CI: 1.12 – 15.29, p=0.033), and bowel wall thickening (adjusted OR 30.67, 95% CI: 6.26 – 150.23, p<0.001) were identified as independent predictive factors. The predictive model showed an AUC of 0.93 (95% CI: 0.88 - 0.98, p=0.001), indicating excellent discriminative ability. Conclusions: Peritoneal signs, the free liquid, and bowel wall thickening are significant predictors of intestinal ischemia in patients with adhesive small bowel obstruction.
