La hemorragia digestiva alta no variceal (HDANV) es una emergencia de alta incidencia y mortalidad. Objetivo: Demostrar que el Sistema PNED tiene mayor validez que la escala de Rockall en la predicción de mortalidad en pacientes con HDANV. Material y métodos: Se realizó un estudio de pruebas diagnósticas, cuya muestra fue de 441 pacientes con diagnóstico de HDANV que ingresaron por emergencia y cumplieron con los criterios de selección durante los años 2020 a 2023 en el Hospital Alta Complejidad Virgen de La Puerta. Resultados: La mortalidad fue de 14.3%. Sistema PNED: Sensibilidad = 87,3%, Especificidad = 91,27%, Valor Predictivo Positivo = 62,5%, Valor Predictivo Negativo = 97,73%, Razón de probabilidad positiva = 10, Razón de probabilidad negativa = 0.14. Escala Rockall: Sensibilidad = 100%, Especificidad = 27.25%, Valor Predictivo Positivo = 18.64%, Valor Predictivo Negativo = 100%, Razón de probabilidad positiva = 1.37, Razón de probabilidad negativa = no pudo calcularse. Conclusión: El Sistema PNED tiene mayor validez que la escala Rockall en la predicción de mortalidad en pacientes con HDANV. |

Therefore, the use of predictive scores would allow directing all medical efforts towards high-risk patients to avoid a fatal outcome. Objective: To demonstrate that PNED score has greater validity than Rockall score in predicting mortality in patients with NVUGIB. Material and methods: A diagnostic test study was carried out, whose sample was 441 patients with a diagnosis of NVUGIB who were admitted for emergency and met the selection criteria during the years 2020 to 2023 at Hospital Alta Complejidad Virgen de La Puerta. Results: Mortality was 14.3%. PNED score: Sensitivity=87.3%, Specificity=91.27%, Positive predictive value =62.5%, Negative predictive value =97.73%, Positive likelihood ratio = 10, Negative likelihood ratio = 0.14.
