Los desenlaces primarios fueron porcentaje de pérdida de peso excesivo, porcentaje de pérdida de peso total y reducción del IMC; los desenlaces secundarios incluyeron complicaciones mayores y menores dentro de los primeros 30 días postoperatorios. Se utilizó la herramienta RoB 2.0 para evaluar el riesgo de sesgo y un metaanálisis con modelos de efectos aleatorios. Resultados: Se incluyeron 11 estudios. La diferencia en la reducción del porcentaje de pérdida de peso excesivo entre ambas técnicas no fue estadísticamente significativa (MD: -3.78%; IC 95%: -8.17 a 1.61; p = 0.11). Sin embargo, LRYGB mostró una mayor pérdida de peso total. No se encontraron diferencias significativas en la reducción del IMC al año (MD: -0.76; IC 95%: -2.10 a 0.58; p = 0.19). La LSG presentó menor riesgo de complicaciones mayores y menores Conclusiones: Ambos procedimientos son eficaces en la reducción de peso, pero el LRYGB mostró una mayor pérdida de peso total. La LSG se asoció con un menor riesgo de complicaciones mayores y menores en el postoperatorio temprano, lo que sugiere una mejor seguridad.

Materials and methods: A systematic review was conducted following the PRISMA 2020 guidelines. Randomized clinical trials identified in PubMed, Scopus, Web of Science, and Embase were included. Both surgical procedures were compared in terms of efficacy and safety. Primary outcomes were percentage of excess weight loss, percentage of total weight loss, and reduction in BMI; secondary outcomes included major and minor complications within the first 30 postoperative days. The RoB 2.0 tool was used to assess the risk of bias and a meta-analysis with random effects models. Results: Eleven studies were included. The difference in the reduction of the percentage of excessive weight loss between both techniques was not statistically significant (MD: -3.78%; 95% CI: -8.17 to 1.61; p = 0.11). However, LRYGB showed greater total weight loss. No significant differences were found in BMI reduction at 1 year (MD: -0.76; 95% CI: -2.10 to 0.58; p = 0.19).
