Resultado: los estudios revisados revelaron que la adherencia al tratamiento antihipertensivo es baja en muchos pacientes. Los principales factores fueron: olvidó tomar el medicamento, poca comprensión del tratamiento, dificultades económicas, poca accesibilidad a los servicios de salud y una comunicación deficiente con el personal médico. También se observó que la adherencia mejora cuando hay apoyo familiar, seguimiento del equipo de salud y estrategias educativas claras. Conclusión: la adherencia al tratamiento es un problema complejo que depende de factores personales, sociales, económicos y del sistema de salud. Mejorarla requiere estrategias integrales, como educación en salud, simplificación del tratamiento, uso de tecnología y fortalecimiento de la relación médico-paciente.

This search used a qualitative, non-experimental, descriptive design. Studies evaluating adherence in patients aged 20 to 85 years diagnosed with hypertension were included. Result: the reviewed studies revealed that adherence to antihypertensive treatment is low in many patients. The main factors were forgetting to take the medication, poor understanding of the treatment, financial difficulties, limited access to health services, and poor communication with medical personnel. It was also observed that adherence improves with family support, follow-up by the healthcare team, and clear educational strategies. Conclusion: treatment adherence is a complex problem that depends on personal, social, economic, and healthcare system factors.
