If you tested negative , was repeated but with ISB 5 mg SL. The same protocol was performed in patients without a history of syncope. RESULTS : 23 patients were studied. In the baseline test , 4 (17%) tested positive , at 19 (83%) were negative ISB stimulated with 5 mg SL : 10 patients had positive response (43% of total). Only one patient had side effects ( severe headache) . Only one control subject (8 %) had PI (+) after the administration of ISB. Most patients (64 %) were under 40 years. The frequency of positive tests going and specificity of the test with ISU SL is 60 % and 92 % , respectively. CONCLUSIONS: PI stimulated ISB has good sensitivity , specificity and positive predictive value is comparable to stimulation with isoproterenol, which is the technique used, and is quick and simple. We recommend its routine use in the diagnosis of recurrent syncope of undetermined origin .OBJETIVO: Iniciar en nuestro medio el estudio del síncope recurrente de origen no determinado em- pleando la Prueba de Inclinación (PI) sensibilizada con dinitrato de isosorbide (ISB). MATERIAL y MÉTODOS: Estudio prospectivo de pacientes con síncope recurrente, seto 1995 - seto 1996, en el Centro Médico Naval, utilizando la PI (80° por 25 min). Si la prueba resultaba negativa, se repetía pero con ISB 5 mg SL. Se realizó el mismo protocolo a pacientes sin antecedentes de síncope. RESULTADOS: Se estudió 23 pacientes. En la prueba basal, 4 (17%) tuvieron resultado positivo; a los 19 (83%) negativos se les estimuló con ISB 5 mg SL: 10 pacientes dieron respuesta positiva (43 % del total). Sólo 1 paciente presentó efecto colateral (cefalea intensa). Sólo 1 sujeto control (8%) tuvo PI (+) luego de la administra- ción de ISB. La mayoría de pacientes (64 %) eran menores de 40 años. La frecuencia de pruebas positi- vas y la especificidad de la prueba con ISU SL es de 60% y 92%, respectivamente. CONCLUSIONES: La PI estimulada con ISB presenta buena sensibilidad, especificidad y valor predictivo positivo, es comparable a la estimulación con isoproterenol, que es la técnica más usada, y es rápida y simple.
