description Objetivo: Determinar si la enzima lactato deshidrogenasa tiene valor como predictor de necrosis intestinal en pacientes con obstrucción intestinal atendidos en el Hospital Regional Docente de Cajamarca. Material y métodos: Se llevó a cabo un estudio retrospectivo seccional de pruebas diagnósticas en el que se incluyeron 163 fichas de registro de pacientes con obstrucción intestinal, según criterios de selección los cuales se dividieron en 2 grupos: fallecidos y sobrevivientes; calculándose el área bajo la curva de la enzima lactato deshidrogenasa. Resultados: El promedio de tiempo de enfermedad y la frecuencia de hiperlactatemia y de leucocitosis fueron significativamente mayores en el grupo de pacientes con necrosis intestinal respecto al grupo sin necrosis intestinal (p<0.05). La sensibilidad y especificidad de la enzima lactato deshidrogenasa como predictor de necrosis intestinal en obstrucción intestinal fue de 85% y 83% respectivamente. El valor predictivo positivo y el valor predictivo negativo de lactato deshidrogenasa como predictor de necrosis intestinal en obstrucción intestinal fueron 59% y 95% respectivamente. La exactitud pronostica de lactato deshidrogenasa como predictor de necrosis intestinal en obstrucción intestinal fue de 75%, siendo el mejor punto de corte el valor de 800 UI/ dl. Conclusión: La enzima lactato deshidrogenasa tiene valor como predictor de necrosis intestinal en pacientes con obstrucción intestinal atendidos en el Hospital Regional Docente de Cajamarca.

Material and methods: A retrospective sectional study of diagnostic tests was carried out in which 163 registration records of patients with intestinal obstruction were included, according to selection criteria which were divided into 2 groups: deceased and survivors; calculating the area under the curve the enzyme lactate dehydrogenase. Results: The average time of illness and the frequency of hyperlactatemia and leukocytosis were significantly higher in the group of patients with intestinal necrosis compared to the group without intestinal necrosis (p <0.05). The sensitivity and specificity of lactate dehydrogenase as a predictor of intestinal necrosis in intestinal obstruction was 85% and 83% respectively. The positive predictive value and the negative predictive value of the enzyme lactate dehydrogenase as a predictor of intestinal necrosis in intestinal obstruction was 59% and 95% respectively. The prognostic accuracy of the enzyme lactate dehydrogenase as a predictor of intestinal necrosis in intestinal obstruction was 75%, with the best cut-off point being the value of 800 IU/dl.
