Resultados: se analizaron 12 estudios (2016–2025; 67% entre 2022–2025). Predominaron diseños transversales y tres ensayos aleatorizados. El 83% vinculó mayor adherencia con mejores desenlaces (supresión viral, retención). Barreras principales: depresión, consumo de sustancias, efectos adversos y desigualdad estructural. Facilitadores: servicios integrales (≈+18% retención), farmacovigilancia y intervenciones mHealth (mensajes/app) con adherencia >90% y mejor supresión viral. Grupos vulnerables: adolescentes y mujeres en edad reproductiva. Conclusión: la adherencia al TAR en América Latina es multifactorial. La combinación de apoyo en salud mental, tecnologías móviles, seguimiento farmacéutico y modelos de atención integrados ofrece el mayor potencial para mejorar la continuidad terapéutica y los resultados clínicos. Se requieren estudios longitudinales y de implementación para evaluar sostenibilidad y costo-efectividad.

Materials and methods: descriptive systematic review (PRISMA 2020) of PubMed, Scopus, ScienceDirect and Epistemonikos. We included observational studies and randomized trials enrolling Latin American populations. Data were extracted with a standardized matrix and synthesized narratively by themes. Results: twelve studies were included (2016–2025; 67% published 2022–2025). Cross-sectional designs predominated plus three randomized trials. Eighty-three percent linked higher adherence with better outcomes (viral suppression, retention in care). Key barriers: depression, substance use, adverse events and structural inequities. Enablers: integrated services (≈+18% retention), active pharmacovigilance, and mHealth (SMS/apps) yielding >90% adherence and improved viral suppression. Vulnerable groups included adolescents and women of reproductive age. Conclusion: ART adherence in Latin America is multifactorial. Integrating mental-health support, mobile technologies, pharmacist-led follow-up, and team-based care models shows the greatest promise to enhance treatment continuity and clinical outcomes.
