Se incluyeron estudios con adultos ≥18 años que evaluaran antidepresivos contemporáneos frente a placebo, antidepresivos convencionales u otras intervenciones. Se analizaron 14 estudios tras aplicar criterios de selección, extrayendo datos de eficacia y seguridad. Resultados: el 93% de los estudios reportó eficacia significativa de fármacos como esketamina, ketamina, lumateperona, seltorexant, aticaprant, brexpiprazol y zuranolona. Destacó la rapidez de acción (24–72 h) de ketamina/esketamina y la eficacia sostenida en depresión resistente o con respuesta inadecuada. Los eventos adversos fueron, en su mayoría, leves o moderados. Solo un estudio mostró eficacia inferior frente a terapia electroconvulsiva. Conclusión: los antidepresivos contemporáneos son intervenciones eficaces y seguras, con ventajas en rapidez de acción y utilidad en casos resistentes. Su perfil terapéutico complementa a los antidepresivos tradicionales y amplía las opciones disponibles para el manejo de la depresión en adultos.

Materials and methods: a systematic review was conducted following PRISMA 2020 guidelines. Searches were performed in PubMed, Scopus, ScienceDirect, and Epistemonikos (2020–2024). Eligible studies included adults ≥18 years receiving contemporary antidepressants versus placebo, conventional antidepressants, or other standard interventions. Fourteen studies met the 4riterio4 4riterio, and data on efficacy and safety outcomes were extracted. Results: ninety-three percent of the studies demonstrated significant efficacy of agents such as esketamine, ketamine, lumateperone, seltorexant, aticaprant, brexpiprazole, and zuranolone. Ketamine/esketamine showed rapid onset of action (24–72 hours), while other agents demonstrated sustained improvements in treatment-resistant or inadequately responsive depression. Adverse events were mostly mild to moderate. Only one study found lower efficacy compared to electroconvulsive therapy. Conclusion: contemporary antidepressants are effective and safe therapeutic options, offering advantages such as rapid symptomatic improvement and clinical utility in resistant cases.
