Materiales y métodos: revisión sistemática cualitativa, descriptiva y transversal, siguiendo PRISMA 2020. Se buscaron ensayos clínicos aleatorizados en PubMed, Scopus, ScienceDirect y Epistemonikos (18–19/11/2025), en inglés y español; se aplicaron criterios PICO y selección a texto completo. Resultados: se identificaron 50 845 registros; tras eliminar duplicados quedaron 4 043; se evaluaron 142 textos completos y se incluyeron 12 ensayos en la síntesis cualitativa. Globalmente, las terapias contemporáneas mostraron resultados favorables en control glucémico (HbA1c/glucosa), con beneficios frecuentes en peso. En poblaciones de mayor riesgo se evidenciaron efectos cardiorrenales (reducción de desenlaces renales y mejoras en parámetros vasculares). La seguridad fue, en general, aceptable; los eventos adversos más reportados fueron gastrointestinales y aumentaron las discontinuaciones con dosis altas. Conclusión: la evidencia 2020–2025 respalda que las terapias antidiabéticas contemporáneas son, en conjunto, eficaces y seguras frente a comparadores en adultos con DM2, con especial utilidad en pacientes con sobrepeso y/o riesgo cardiorrenal.

Randomized controlled trials were searched in PubMed, Scopus, ScienceDirect and Epistemonikos (18–19 Nov 2025) in English and Spanish; PICO criteria and full-text selection were applied. Results: 50,845 records were identified; after deduplication, 4,043 remained; 142 full texts were assessed and 12 trials were included in the qualitative synthesis. Overall, contemporary therapies showed favorable glycemic outcomes (HbA1c/blood glucose), frequent weight benefits, and cardiorenal effects in higher-risk populations (reduced renal endpoints and improved vascular parameters). Safety was generally acceptable; the most commonly reported adverse events were gastrointestinal, with higher discontinuation rates at higher doses.
