description This case report describes the orthopedic-orthodontic treatment of a 15-year-old adolescent attended the Dental Center at the University of San Martín de Porres, indicating that his chief complaint was the appearance of his smile. The patient presented a straight profile with a protruding lower lip and a class III skeletal malocclusion (ANB, -2 °), with class III occlusal relations, moderate-severe crowding, right posterior and anterior crossbite, and dentoalveolar compensations. The patient accepted an orthopedic treatment with maxillary expansion and protraction to manage the residual potential growth accompanied by orthodontic camouflage with extractions of the second upper premolars and first lower premolars. Treatment was started using a McNamara type expander, with an alternate expansion and constriction protocol for 7 weeks and was combined with the use of a Petit facial mask for 6 months. Meanwhile, the lower premolars were removed and the canines were retracted using a sectioned arch with conventional brackets. Subsequently, the rest of the fixed appliances were installed to align and level the dental arches. Once the cross bites were solved, the upper premolars were extracted and the spaces were closed with chains and Class III elastics. The total active treatment time was 36 months and it was possible to obtain an improvement of the skeletal pattern to a class I (ANB, 1 °), aligned and leveled dental arches together with a mutually protected occlusion. Follow-up after 20 months indicated that the results of the treatment were quite stable.

Follow-up after 20 months indicated that the results of the treatment were quite stable. El presente reporte de caso describe el tratamiento ortopédico-ortodóntico de un adolescente de 15 años atendido en el Centro Odontológico de la Universidad de San Martín de Porres, su queja principal era la apariencia de su sonrisa. Presentaba un perfil recto con labio inferior protruido y una maloclusión Clase III esqueletal (ANB, -2°), con relaciones oclusales Clase III, apiñamiento moderado-severo, mordida cruzada anterior y posterior derecha, y compensaciones dentoalveolares. El paciente aceptó un tratamiento de ortopedia con expansión y protracción maxilar para manejar el potencial de crecimiento residual acoplado a un camuflaje ortodóntico con exodoncias de los segundos premolares superiores y primeros premolares inferiores. Se inició el tratamiento utilizando un disyuntor tipo McNamara, con un protocolo alternado de apertura y cierre durante 7 semanas, con el uso de una máscara facial de Petit por 6 meses. De manera conjunta se extrajeron los premolares inferiores y se retrajeron los caninos mediante un arco seccionado con brackets convencionales. Posteriormente se instaló el resto de la aparatología fija para alinear y nivelar las arcadas dentarias. Una vez que las mordidas cruzadas fueron corregidas se realizaron las extracciones de premolares superiores y se procedió al cierre de espacios con cadenas de poder y elásticos Clase III. El tiempo total de tratamiento activo fue de 36 meses y se logró obtener una mejora del patrón esqueletal a Clase I (ANB, 1°) y arcadas dentarias alineadas y niveladas con una oclusión mutuamente protegida.
