The radiographic and tomographic examination showed that part # 38 is in an inverted position, so it was decided to carry out the extraction of the inverted third lower molar since they were free of pathology and by orthodontic indications. indications of orthodontics. At 8 days after the surgical procedure, the stitches are removed and soft tissue healing, absence of pain, edema and infection are clinically observed. The impact of the lower third molars inverted up to the present has been very little evidenced by finding very few cases on this subject, for this reason we believe that our clinical case would be an important help on the diagnosis and surgical treatment of this type of dental impacts.El objetivo de este estudio es explicar la exodoncia quirúrgica del tercer molar inferior en posición invertida. El presente caso trata de un paciente masculino de 30 años de edad, sin antecedentes médicos relevantes quién asistió a la clínica odontológica SERODU. Al examen radiográfico y tomográfico se observó la pieza #38 en posición invertida, por lo que se decidió llevar acabo la exodoncia del tercer molar inferior mediante incisión triangular y odontosección transversal, dicha pieza se encuentra libre de patología y debe ser retirada por indicaciones ortodóncicas. A los 8 días del procedimiento quirúrgico se retira los puntos de sutura y se observa clínicamente la cicatrización de tejidos blandos, ausencia de dolor, edema e infección.
