Neurotoxicity was defined as the presence of confusion, hallucinations, delirium, or other neurological symptoms that were resolved after discontinuing or substituting the antibiotic. Medical history, renal function, serum albumin, and hemoglobin levels were analyzed, and the Naranjo Scale was applied to assess the probability of causality. Results: Patients were over 60 years old with comorbidities such as hypertension (85.7%), chronic kidney disease (71.4%), and diabetes mellitus (57.14%). Neurological symptoms appeared on average 5.7 days after starting treatment, with confusion and psychomotor agitation being the most frequent. The average duration of ertapenem use was 7 days, and symptoms took 4.1 days to resolve after drug discontinuation. All cases were classified as "probable" according to the Naranjo Scale. Conclusion: Ertapenem-induced neurotoxicity was related to factors such as renal disease, hypoalbuminemia, and advanced age. Dose adjustment and monitoring of risk factors are recommended to minimize complications and optimize its use in clinical practice.

Material y métodos: Se realizó un estudio retrospectivo tipo serie de casos. Se incluyeron siete pacientes atendidos entre los años 2023 y 2024. La neurotoxicidad se definió como la presencia de confusión, alucinaciones, delirio y otros síntomas neurológicos que remitieron tras suspender o sustituir el antibiótico. Se analizaron antecedentes médicos, función renal, albúmina y hemoglobina séricas, además de la Escala de Naranjo para estimar la probabilidad de causalidad entre la reacción adversa y el uso de ertapenem. Resultados: Los pacientes fueron mayores de 60 años con comorbilidades como hipertensión arterial (85,7 %), enfermedad renal crónica (71,4 %) y diabetes mellitus (57,14 %). Los síntomas neurológicos aparecieron en promedio a los 5,7 días de iniciado el tratamiento, siendo la confusión y la agitación psicomotriz los más frecuentes. La duración promedio del uso de ertapenem fue de 7 días, y los síntomas tardaron 4,1 días en resolverse tras la suspensión del medicamento. Los casos fueron clasificados como “probable” según la Escala de Naranjo. Conclusión: La neurotoxicidad por ertapenem se asoció a factores como enfermedad renal, hipoalbuminemia y edad avanzada.
