It was a patient diagnosed with abdominal septic shock of pelvic origin due to vaginal ring infection. In the intensive care unit, the patient was treated with norepinephrine for 5 days. This catecholamine stimulates the adrenergic receptors, causing vasoconstriction and increased blood pressure. After ten days of treatment, the patient presents with ischemic necrosis of all fingers and toes, and amputation is necessary, with aesthetic and functional sequelae.Se presenta el caso de una paciente mujer de 39 años de edad, que en octubre del 2016 fue atendida en nuestro centro (Hospital Dr. Negrin, Islas Canarias-España), con necrosis de todos los dedos de las manos y los pies, referida al servicio de Cirugía Plástica desde la Unidad de Medicina Intensiva. Se trataba de una paciente con diagnostico de shock séptico abdominal de origen pélvico por infección de anillo vaginal. En la unidad de cuidados intensivos, la paciente fue tratada con norepinefrina durante 5 días. Esta catecolamina estimula los receptores adrenérgicos, causando vasoconstricción y el aumento de la presión arterial.
