En la Fase hemática, recientemente se ha demostrado que la Bartonella bacilliformis, agente etiolóqico de la Enfermedad, se encuentra dentro de los hematíes del huésped. En la Fase histioide se describe dos períodos: 1) Período pre-eruptivo y 2) Período eruptivo o brote verrucoso. El cuadro histológico del botón verrucoso es el de un hemangioma capilar o de un granuloma pióqeno: sin embargo, en ocasiones el diagnóstico diferencial con otras entidades, especialmente con neoplasias malignas, es difícil. Con técnicas especiales de coloración, se ha observado en el citoplasma de células histioides del verrucoma gránulos pequeños que han sido considerados como forma de B. bacilliformis. En el presente trabajo se da a conocer los resultados del estudio ultraestructural del complejo Bartonella-hematíe y del verrucoma para lo cual se han usado secciones finas de sangre coagulada, de sangre desfibringda, de hematíes hemolizados y de verrucomas floridos no complicados. Se establece que en el verrucoma existen las tres formas de B. bacilliformis descritas clásicamente en los hematíes, es decir, formas bacilares, cocobacilares y cocoides, considerando que esta última forma, desde el punto de vista ultraestructural, es predominantemente ovalada. La B. bacilliformis, en los hematíes, tiene las características de bacteria con pared gruesa como la descrita en gérmenes gram-poitivos; sin embargo, la B. bacilliiormis del verrucoma, siendo ultraestructuralmente similar, tiene pared bacteriana delgada, diferencia que es discutida en cuanto a su posible significado. De otro lado, las diferentes formas de B. bacilliformis del verrucoma son más pequeñas que sus homólogas del hematíe, considerándose que esta diferencia puede ser artificial, dependiente de una mayor o menor retracción de los tejidos frente a los re activos usados en la preparación de las inclusiones para la obtención de secciones finas. Por las observaciones realizadas deduce que Ia B. bacilliformis es incorporada en el seno del glóbulo rojo por pinocifosis. Este germen se multiplica en el hematíe y en el citoplasma de algunas células del verrucoma por tabicamiento completo transversal. El verrucoma florido no complicado está constituido por varios tipos de células que, probablemente, se originan de la célula más primitiva; tres de ellas, entre las que figura la célula endotelial, tienen capacidad para fagocitar bartonellas, siendo el macrófago el que ejerce mayor actividad fagocitaria. Entre los elementos celulares del verrucoma se observa "sustancia fundamental", detritus celulares, fibras de colóceno y bartonellas. El macrófago del verrucoma se caracteriza por poseer escasos lisomas, hecho que se discute en relación con las características del complejo B. bacilliformis Verrucoma.
