The question is what is the most effective strategy to prevent and treat hypotension secondary to spinal anesthesia for caesarean section? It is crucial in obstetric anesthesia and the response is related to the physiological events that lead to hypotension. For many years the administration of a volume of crystalloids prior to the placement of spinal anesthesia was mandatory, just as ephedrine was considered the vasopressor of choice to treat such hypotension. The available scientific evidence shows us the opposite2.Sr. Editor, La anestesia espinal se considera la técnica de elección en la mayoría de cesáreas, siendo la hipotensión el efecto adverso más común. Los síntomas que frecuentemente acompañan a la hipotensión son: nauseas, vómitos, disnea y efectos deletéreos neonatales, entre ellos, score de Apgar bajo y acidosis en la gasometría del cordón1. La pregunta ¿cuál es la estrategia más efectiva para prevenir y tratar la hipotensión secundaria a la anestesia espinal para cesárea? es crucial en la anestesia obstétrica y la respuesta se encuentra relacionada a los eventos fisiológicos que llevan a la hipotensión. Durante muchos años la administración de un volumen de cristaloides previo a la colocación de la anestesia espinal era mandatoria, así como la efedrina era considerada el vasopresor de elección para tratar dicha hipotensión.
