Zidovudine (AZT) is the drug with most cumulated clinical. The Instituto Materno Perinatal in Lima, Perú, has roughly 24 000 deliveries per year, and is pioneer in using AZT (shorted course) in pregnant women for prevention of vertical transmission of HIV OBJECTIVES: To determine the vertical transmission rate of HIV in our institute, and the impact of the preventive use of AZT. MATERIAL AND METHODS: The study included 113 children of HIV infected mothers, bom and controlled in the period 1998 to 2001. Diagnosis of perinatal infection was basedon the polymerase chain reaction (PCR) test in children older than 6 months, and on the ELISA test in children older than 18 months. RESULTS: The vertical transmission rate in the group treated with AZT was 5,5% and 29,2 % in the untreated group. CONCLUSIONS: There was a protective effect of the antiretroviral therapy (OR: 0, 14, CI95%) with regards to those who were not treated due to delayed postnatal diagnosis. Infection risk decreased from 1/3 to 1/20 in children of mothers treated with AZT.ANTECEDENTES: El SIDA en niños menores de 15 años se adquiere principalmente por transmicion vertical del virus de la inmunodeficiencia humana tipo 1 (VIH). El conocimiento acerca de patogenesis de stta infección a aumentado progresivamente y se han propuesto diversos ensayos terapéuticos para tratar de disminuir la carga viral materna y prevenir la transmisión vertical del virus. La zidovudina (AZT) es el farmaco con mayor experiencia clínica acumulada. El Instituto Materno Perinatal en Lima, Perú, atiende alrededor de 24 000 partos al año y es el pionero en el uso de la AZT en gestantes para la prevención de la transmision vertical del VIH. OBJETIVOS: Determinar la tasa de transmisión vertical del VIH en nuestro Instituto y el impacto del uso preventivo de la AZT (curso acortado). MATERIAL Y MÉTODOS: Comprendió 113 niños de madres infectadas por el VIH, nacidos y controlados en el período 1998 al 2001. El diagnóstico de infección perinatal se basó en la prueba de la reacción de cadena polimerasa en niños mayores de 6 meses. RESULTADOS: La tasa de transmisión vertical en el grupo tratado con AZT fue de 5,5% y 29,2% en el grupo no tratado. CONCLUSIONES: Se demuestra un efecto protector de la terapia antirretrovial (OR: 0, 14, IC, 95%) con respecto a aquellos que no fueron tratados, debido a un diagnostico posnatal tardío.
