Ultrasound showed a 35 x 26 x 30 mm cystic lesion at the frontoparietal level of the interhemispheric midline. In addition, the newborn had Down syndrome with cardiomegaly, interatrial communication, heart failure, respiratory failure, and severe hepatomegaly. He died at 2 days of life. Pathology findings confirmed GVA. We highlight the importance of the Doppler study in a prompt diagnosis in order to initiate an early treatment with the aim of improving life expectancy.Se reporta el caso de un recién nacido de 41 semanas de edad gestacional que presentó un aneurisma de la vena de Galeno, asociado a leucomalacia periventricular y ventriculomegalia. Fue diagnosticado mediante ultrasonido cerebral Doppler color. La imagen ecográfica mostró una lesión quística de 35 x 26 x 30 mm a nivel frontal posterior/parietal de la línea media interhemisférica, con flujo sanguíneo en su interior. El recién nacido además era portador de síndrome de Down, cardiomegalia con comunicación interauricular, hepatomegalia severa e insuficiencias cardiaca y respiratoria. Falleció a los dos días de vida. Los hallazgos anatomopatológicos confirmaron el aneurisma de la vena de Galeno.
