The mechanismsby which nontyphoidal Salmonella causes urinary tract infection are generally attributed tohematogenous spread resulting from gastroenteritis or to the introduction of fecal microbiotathrough the urethra. In this case, the patient presented with a clinical picture characterized bythe onset of watery stools accompanied by emesis and followed by urinary symptoms.The presented clinical features prompted the development of this case report, which aims toprovide guidance in identifying risk factors and clinical manifestations that may raise suspicion ofSalmonella as the etiology. The patient had a history of acute gastroenteritis, after which urinarysymptoms and septic shock developed. The bacillus was isolated in both blood and urine culturesand was found to be sensitive to ciprofloxacin, the first-line antibiotic due to its intracellularactivity. This case underscores the importance of rational antibiotic use in the light of theemergence of beta-lactamase-producing Salmonella in Paraguay.This case report is noteworthy due to the infrequency of urinary tract infections with septic shockcaused by Salmonella. In this context, it is important not only to know the clinical syndromes thatmay be attributed to this bacillus, but also to highlight the patient’s socioeconomic background,since the infection is associated with the consumption of contaminated liquid or food.

Afecta predominantemente a niños y a personas mayores. Los mecanismos por los cuales la Salmonella no Typhi establece una infección en el tracto urinario se atribuyen, por lo general, a la diseminación hematógena provocada por la gastroenteritis o a la introducción de flora fecal por la uretra. En este caso, el paciente presentó una clínica caracterizada por la aparición de deposiciones líquidas acompañadas de emesis, tras lo cual se manifestaron los síntomas urinarios. Las características presentadas motivaron el desarrollo de este reporte de caso, cuyo objetivo consiste en servir de orientación para identificar los factores de riesgo y las manifestaciones clínicas que permitan sospechar de la Salmonella como etiología. El paciente presentó antecedente de gastroenteritis aguda; posteriormente, aparecieron los síntomas urinarios y el shock séptico. Se aisló el bacilo en el hemocultivo y urocultivo, sensible a ciprofloxacino, antibiótico de primera elección por su efecto intracelular. Se destaca la necesidad del uso racional de antibióticos ante la presencia de Salmonella productora de betalactamasa en Paraguay. El caso clínico resulta de interés debido a la poca frecuencia de infecciones del tracto urinario con shock séptico causado debido a la Salmonella. Ante esto, resulta importante saber no solo los síndromes clínicos que puedan atribuirse a dicho bacilo, sino resaltar el aspecto socioeconómico del paciente, ya que la infección está relacionada con el consumo de líquidos o alimentos contaminados.Universidad de San Martín de Porres.
