We review the data of the ultrasound report from 3 562 neonates, since august 1999 until august 2004. The transfontanellar ultrasound studies were performed in the Diagnostic Imaging and Neonatal Intensive Care Services. An ultrasound equipment with micro-convex transducer of 5 and 7.5 Mhz was used. Results: there were 8 (0,2%) cases of thalamic damage: 6 unilateral and 2 bilateral, 5 in preterm neonates and 3 in term neonates. The average weight of the preterm newborns was 1,490 gr, and in the term newborns was 3,210 gr. The average gestational time in newborns was 30,4 weeks and 39,3 weeks for the term ones. The prenatal factors were: asphyxia (4), coagulopathy (3), premature detachment of the placenta (1), prolapse of umbilical cord (1), and undetermined (3). Five out 5 neonates died. Conclusion: the thalamic damage is a rare presentation, and it was associated fundamentally to the severe hypoxic-isquemic encephalopathy and coagulopathy problems. There were two ways of presentation: bilateral thalamic damage in term neonates and unilateral thalamic damage in preterm neonates.Objetivos: Determinar las características ultrasonográficas de la lesión talámica neonatal (hemorragia o infarto) y de sus factores perinatales asociados. Material y Métodos: Es un estudio descriptivo retrospectivo de una serie de casos de neonatos con lesión talámica unilateral y bilateral. Se revisaron los informes ecográficos de 3 562 neonatos, de agosto 1999 hasta agosto 2004. Los exámenes ecográficos transfontanelares se realizaron en el servicio de Cuidados Intensivos Neonatales y Servicio de Diagnóstico por Imágenes, se utilizó un ecógrafo con transductores microconvexos de 5 y 7.5MHz. Resultados: Se presentan 8 (0,2%) casos de lesión talámica: 6 unilaterales y 2 bilaterales; 5 en neonatos pretérmino y 3 en neonatos a término. El peso promedio de los prematuros fue de 1,490gr y el de los recién nacidos a término 3,210 gr. La edad gestacional promedio en prematuros fue 30,4 semanas y en los neonatos a término 39,3 semanas. Los factores perinatales asociados fueron: asfixia (4), coagulopatía (3), desprendimiento prematuro de placenta (1), prolapso de cordón (1) y no determinada (3). Mortalidad en 5 de 8 neonatos. Conclusión: la lesión talámica es de presentación rara, esta asociada principalmente a encefalopatía hipoxica-isquémica severa y a trastornos de la coagulación.
