There is endothelial cell dysfunction, dyslipidemia (especially hypertriglyceridemia), hyperinsulinemia, hyperleptinemia, antioxidant-prooxidant imbalance. Preeclampsia is an early pregnancy disease characterized by hypertension and proteinuria, expressed late in pregnancy. We still do not have a clinical useful test to predict preeclampsia. Combining both placental insufficiency and endothelial markers and PlGF y sFlT-1 serum or urinary levels in the first trimester is promising. Personal and familiar hypertension history is extremely important. Pregnant women with arterial hypertension should be attended in specialized centers. The antihypertensive therapy has little effect on pregnancy outcome. We suggest the use of low doses of aspirin in women with previous early preeclampsia. Calcium channel blockers are very useful in treating acute hypertension. Preeclampsia treatment consists in the birth of both fetus and placenta, the time determined by maternal and fetal conditions.El factor de inicio en la preeclampsia puede ser la reducción de la perfusión uteroplacentaria, secundaria a una anomalía en la invasión de las arterias espirales uterinas por las células citotrofoblásticas extravillosas, asociada a alteraciones locales del tono vascular, del balance inmunológico y del estado inflamatorio, algunas veces con predisposición genética. Se encuentra disfunción de la célula endotelial, dislipidemia (especialmente, hipertrigliceridemia), hiperinsulinemia, hiperleptinemia, disbalance entre antioxidantes y prooxidantes, entre otros. La preeclampsia es una enfermedad temprana del embarazo, caracterizada por hipertensión y proteinuria, que se expresa tarde en el embarazo. Aún no existe una prueba clínicamente útil para predecir el desarrollo de la preeclampsia. Son promisorias el combinar marcadores de insuficiencia placentaria y de función endotelial, y los niveles séricos o urinarios de PlGF y sFlT-1 en el primer trimestre. Tan o más importante es la historia clínica personal y familiar de hipertensión. Las gestantes con hipertensión arterial deben ser manejadas en centros especializados. La terapia antihipertensiva tiene poco efecto sobre el progreso del embarazo. Se sugiere administrar aspirina a dosis bajas en mujeres con preeclampsia previa de inicio precoz. Los bloqueadores de canales de calcio son muy útiles en el manejo de la hipertensión aguda.
