After 8 days being hospitalized we decided to perform surgery in this patient, because of the development of peritoneal signs. We performed an exploratory laparotomy, where a perforation 40-cm distant from the ileo-cecal valve was found, surrounded by a tumoral mass. The lesion was excised and the pathology examination showed the presence of chronic inflammation and tissue repair, with a severe lymphocyte, plasma cells and macrophage infiltration was found, compatible with histoplasmosis. This condition was confirmed when the samples were stained using PAS. Intestinal histoplasmoma is a pseudotumoral lesion that may be found in patients with disseminated histoplasmosis and immune suppression.Se reporta el caso de un paciente varón de 23 años, con antecedente de ser portador del Virus de Inmunodeficiencia Humana (VIH), quien acude a Emergencia con dolor abdominal, diarrea crónica y pérdida de peso de 2 meses de evolución. En la radiografía de abdomen se observó distensión de asas intestinales con niveles hidro-aéreos dispersos, la ecografía de abdomen mostró meteorismo intestinal y líquido libre en cavidad. Después de 8 días de hospitalización se decide optar por intervención quirúrgica por presentar signos peritoneales, se realizó laparotomía exploratoria evidenciándose una perforación a 40 cm de válvula ileocecal cuyos bordes impresionaron como una lesión tumoral, se resecó la perforación. En el estudio anatomopatológico se informa proceso inflamatorio crónico y reparación tisular prominente con severo infiltrado de linfocitos, plasmocitos y gran número de macrófagos parasitados, compatible con Histoplasmosis. La coloración Ácido Peryódico de Schiff (PAS) confirmó el diagnóstico.
