Sumario:Patients with a malignancy are at high risk of suffering from not only a second but also a third synchronous primary malignancy. This singular event, characterized by the association of three or more malignancies simultaneously, is infrequent, with a frequency ranging from 0.04% to 0.81%, and it predominantly affects women. The etiology is based on a strong association with genetic instability (BRAF, RAS, etc.), which interacts and leads to unfavorable outcomes in the prognosis of survival. We present the case of a 77-year-old man who underwent initial surgery for papillary thyroid carcinoma, colon adenocarcinoma, and clear cell renal carcinoma simultaneously, who was diagnosed and managed with initial surgery. After tolerating three interventions to date, the patient is being monitored without evidence of recurrence and is evolving favorably, without alterations in lifestyle, which suggests that surgery has high potential in the management of these neoplasms.
