Oroya fever and Peruvian wart on one disease. Peruvian wart is an infection with a characteristic clinical pattern, caused by Bartonella bacilliformis. Recent published studies awarded as the cause of a new Peruvian wart bartonella, the Candidatus Bartonella ancashi. Analyzing research, the proposed causality of B. ancashi is based only on observational evidence: the association of the new bartonella in one patient with a clinical pattern similar to the wart Peruvian and the origin of the patient from an endemic area. With the new B. ancashi has not been investigated whether or not it meets all of Kochs postulates, still lack experimental reproduction of Peruvian wart animals to contend with more proven that causes the Peruvian wart. Observational evidence does not necessarily determine causality. An association between a specific infection and a particular infectious agent does not means that the agent cause disease. Science needs to base its conclusions on the best available evidence and reasoning.La mayoría de las bacterias que causan enfermedades en el ser humano se adaptan a los clásicos postulados de Koch. Los postulados de Koch tuvieron como primer modelo al Bacillus anthracis, la primera bacteria que se demostró ser causante de una enfermedad infecciosa y cuyo ciclo biológico fue descubierto por Koch. La fiebre de La Oroya y la verruga peruana son una sola enfermedad, y la Bartonella bacilliformis es la causante de esta. Recientes estudios publicados adjudican como causa de la verruga peruana a una nueva bartonela, la Candidatus Bartonella ancashi. Si se analiza la investigación, se nota que la causalidad propuesta de la B. ancashi está basada solo en evidencia observacional: la asociación de la nueva bartonela en un solo paciente con un patrón clínico similar al de la verruga peruana y la procedencia del paciente de una zona endémica. Con la nueva B. ancashi no se ha investigado si cumple o no con todos los postulados de Koch; aún falta la reproducción experimental de la verruga peruana en animales para afirmar con mayor solidez científica que causa la verruga peruana. La evidencia observacional no determina necesariamente causalidad. Una asociación entre una determinada infección y un agente infeccioso particular no significa que el agente cause la enfermedad.
