Tofacitinib es un inhibidor de la cinasa Janus oral para el tratamiento de la AR. Este análisis post hoc evaluó la seguridad de tofacitinib en pacientes latinoamericanos (LA) con AR frente a la población del resto del mundo (RoW). Métodos: Se combinaron los datos de 14 estudios clínicos de tofacitinib: seis de fase 2, seis de fase 3 y dos estudios de extensión a largo plazo. Se calcularon las tasas de incidencia (RI; pacientes con eventos / 100 pacientes-año de exposición al tratamiento) para los eventos de seguridad de especial interés combinados entre las dosis de tofacitinib. Los intervalos de confianza (IC) del 95% para los RI se calcularon utilizando el método de máxima verosimilitud. Se realizaron comparaciones descriptivas entre las poblaciones de LA y RoW (excluyendo LA). Resultados: Este análisis incluyó datos de 984 pacientes de LA y 4687 pacientes de RoW. Los RI para eventos de seguridad de interés especial fueron generalmente similares entre las poblaciones de LA y el DdV, con IC del 95% superpuestos. Los RI para la interrupción debido a eventos adversos, infecciones graves, tuberculosis, todos los herpes zoster (HZ), HZ graves, neoplasias malignas (excluyendo el cáncer de piel no melanoma) y eventos cardiovasculares adversos mayores fueron numéricamente más bajos para los pacientes con AL que con el DdV; La RI de mortalidad fue numéricamente más alta. No se informó ningún linfoma en la población de Los Ángeles frente a ocho casos en la población del DdV. Methods Data were pooled from 14 clinical studies of tofacitinib: six Phase 2, six Phase 3 and two long-term extension studies. Descriptive comparisons were made between LA and RoW (excluding LA) populations. Results This analysis included data from 984 LA patients and 4687 RoW patients. IRs for safety events of special interest were generally similar between LA and RoW populations, with overlapping 95% CIs. IRs for discontinuation due to adverse events, serious infections, tuberculosis, all herpes zoster (HZ), serious HZ, malignancies (excluding non-melanoma skin cancer) and major adverse cardiovascular events were numerically lower for LA versus RoW patients; IR for mortality was numerically higher. No lymphoma was reported in the LA population versus eight cases in the RoW population. Exposure (extent and length) was lower in the LA population (2148.33 patient-years [mean = 2.18 years]) versus RoW (10515.68 patient-years [mean = 2.24 years]).
