Fetal surgery: Current situation

Fetal surgery, or better called maternal-fetal, because drugs and surgical techniques are administered to the fetus through the mother, therefore, share risks, began more than 40 years ago. In 2016, on the subject of a symposium on the relevance of fetal medicine in Peru, published in this journal, we affirmed: "These advances make up a new multidisciplinary medical specialty, which considers the fetus as an individual patient and in the relationship with its mother. , establishing prophylactic treatments, direct medical therapy or through the mother, interventions guided by ultrasound, open and fetoscopic surgeries, with advantages to conventional treatments. This reality obliges us to review ethical and legal aspects related to the unborn, to changes both in the organization of health services, and in the pre and postgraduate curricula of new professionals ". We added: "Currently, in our country, the application of these advances is in the incipient stage. At the decision levels of health policies there is ignorance and disinterest in the subject despite being a priority. However, academic and scientific institutions insist on the need to create a formal organization within the health system that guarantees quality Fetal Medicine "(1).
La cirugía fetal -o mejor denominada materno-fetal, porque los medicamentos y técnicas quirúrgicas son administrados al feto a través de la madre, por tanto, comparten riesgos-, se inició hace más de 40 años. El 2016, a propósito de un simposio sobre actualidad de la medicina fetal en el Perú, publicado en esta revista, afirmamos: “Estos avances configuran una nueva especialidad médica multidisciplinaria, que considera al feto como un paciente individual y en la relación con su madre, estableciéndose tratamientos profilácticos, terapéutica médica directa o través de la madre, intervenciones guiadas por ecografía, cirugías abiertas y fetoscópicas, con ventajas a los tratamientos convencionales. Esta realidad obliga a revisar aspectos éticos y legales relacionados al no nacido, a cambios tanto en la organización de los servicios de salud, como en la currícula del pre y posgrado de los nuevos profesionales”. Añadíamos: “Actualmente, en nuestro país, la aplicación de estos avances está en etapa incipiente. En los niveles de decisión de las políticas de salud hay desconocimiento y desinterés por el tema a pesar de ser prioritario. Sin embargo, a través de las instituciones académicas y científicas se insiste en la necesidad de crear una organización formal dentro del sistema de salud que garantice una Medicina Fetal de calidad”(1).
