In 73 patients (16,66%) colposcopy was positive and a directed biopsy was performed. In 66 of these patients (90,41%), the histologically findings were: 20 (30,30%) low grade squamous intraepithelial lesions; 35 (53,03%) high grade lesions and in 11 patients (16,66%) invasive cancer. Sexual relations before 18 years, more than 3 sexual partners and the antecedent of sexual transmitted infections were risk factors; but cigarette smoking, low social status and the use of contraceptive methods, were not. In 22 patients (33,33%) histologically changes produced by H.P.V. infection were found and 12 patients (18,18%) had positive vulvar biopsy for H.P.V. In 27 patients the cytologic smear was negative. False negative cytology in this study was 40,90%.Las lesiones preneoplásicas y neoplásicas de cuello uterino se asocian en elevado porcentaje a infección por papiloma virus humano (H.P.V.). Para su detección es necesario el uso conjunto de citología, colposcopia y biopsia dirigida. En el período comprendido entre febrero de 1994 a setiembre 1995, se evaluó en el departamento académico de la Universidad de San Agustin y Hospital Honorio Delgado de Arequipa, 438 pacientes, siguiendo la metodología diagnóstica antes mencionada. En 73 pacientes (90,14%) se encontró histológicamente: 20 (30,30%) lesiones intraepiteliales escamosas de grado bajo; 35 (53,03%) de grado alto y en 11 pacientes (16,16%) cáncer invasor. El I.R.S. antes de los 18 años, tener 3 ó más parejas sexuales y antecedente de enfermedades de transmisión sexual, constituyeron factores de riesgo. En cambio la paridad, uso de anticonceptivos, tabaquismo y nivel socioeconómico bajo no lo fueron. En 22 pacientes (33,33%) se encontró signos histomorfológicos de infección por H.P.V. En 12 pacientes (18,18%) la biopsia de vulva reveló lo mismo. 27 pacientes tuvieron una citología negativa.
