The medical profession, influenced by various historical and contemporary factors, is in constant evolution. Technological advances, social changes, economic factors, and scientific developments have shaped the perception of physicians in society. Ethical self-regulation, along with high-quality medical education, is essential for preparing competent and ethical professionals. 21st-century physicians play a crucial role in healthcare, health education, and health promotion. Despite challenges such as monopoly and elitism, it is argued that these do not necessarily diminish the prestige of the profession and can be ethically addressed. El internado médico en nuestro país representa la época de consolidación del conocimiento y aplicación del mismo en un ambiente clínico estructurado; por ejemplo, un centro de salud enfocado en el primer nivel de atención o un centro médico especializado. El portafolio está conformado por treinta casos clínicos con aprendizajes significativos durante el internado médico en el Centro Materno Infantil “El Porvenir” y el Hospital Nacional Dos de Mayo. Consiste en un breve relato médico, diagnóstico, complicaciones, tratamiento y las reflexiones de cada caso clínico. Failed back surgery syndrome is a postsurgical phenomenon that involves persistent low back pain following one or more spinal surgeries. The multidisciplinary approach to its management includes both conservative and surgical treatments. Anterior lumbar interbody fusion (ALIF) has stood out as a surgical option for refractory pain. We present five cases of patients with failed back surgery syndrome treated with ALIF. These patients demonstrated reduced pain scale scores and significant improvement in functional capacity after the application of the abovementioned technique, suggesting its effectiveness in this group of patients who are refractory to conservative treatments.
