Objective: To compare morbidity and mortality in patients undergoing coronary artery bypass graft (CABG) surgery with acute myocardial infarction (AMI) and electively (non-AMI). Methods: Retrospective cohort study. Medical records of 237 patients who underwent CABG for three-vessel and/or left main disease were reviewed. 63 patients had surgery during AMI, and 174 had elective surgery. Morbidity and mortality were compared during the hospital stay or within 30 postoperative days. Results: Clinical characteristics were similar in both groups. Operative mortality was higher in the AMI group (7.94% vs. 1.15%, p = 0.006). Common postoperative complications in the AMI group were: stroke (7.94% vs. 0.57%, p = 0.001), reoperation for bleeding (6.35% vs. 1.15%, p = 0.024), and acute kidney injury (15.2% vs. 3.5%, p = 0.009). Se presenta el caso de un varón de 78 años con estenosis carotídea izquierda del 80-95% asintomática, localizada en una posición alta, por encima de la segunda vértebra cervical, tratado con la técnica de revascularización transcarotídea (TCAR, por sus siglas en inglés: “Transcarotid Arterial Revascularization”). En este procedimiento se utilizó un equipo deneuroprotección de flujo reverso, el cual fue adaptado utilizando insumos comúnmente disponibles en sala de operaciones. Se describe en detalle tanto el caso como la preparación del equipo.

We present the case of an 82 years old male with Congestive Heart Failure (Left Ventricle Ejection Fraction: 20%) and Acute Pulmonary Edema secondary to Acute Myocardial Infarction that required Coronary Artery Bypass Graft (CABG) surgery. Past Medical History included Aortoiliac Occlusive Disease with total occlusion of the Left Iliac Artery and collateral circulation to the left inferior limb through the Left Internal Mammary Artery (LIMA).  CABG was perfomed without harvesting the LIMA due to high risk of legischemia. An attempt to revascularize the left limb previous to the cardiac surgeryin other to do so. Would have been too risky in this case. After 2 months follow up, patient showed good clinical outcomes.
