Introduction. The care of the neonate in critical condition is a complex situation that requires of training and continuing education, through the incorporation of active methodologies of learning, as the clinical simulation. Objective: To describe the therapeutic results of patients treated at the Neonatology Unit of the Instituto Nacional de Salud del Niño San Borja, Lima, Peru. Methods: A descriptive study was carried out where the information of the patients who attended between the years 2018 and 2022 was used, by reviewing their medical records. Results: During the evaluation period, a total of 1,134 patients were attended, with a median age of 6 days, and the majority were born outside of Lima. 92.2% received a surgical diagnosis, with structural heart defects, anorectal malformations, and small bowel atresia among the most prominent. In general, survival to discharge from our intensive and intermediate care neonatology unit was 93.7%, with an unequal distribution of deaths in patients who required transfer from cities in Peru other than Lima. La atención del recién nacido ocupa un lugar importante en la agenda pública dentro de los objetivos del desarrollo sostenible, ya que busca reducir la tasa de muertes evitables y disminuir la morbilidad, debido a las consecuencias significativas que puede tener en el bienestar y desarrollo a largo plazo. La mortalidad neonatal refleja el nivel de cuidado prenatal y atención al recién nacido en una región determinada, sirviendo como indicador del impacto de los programas de intervención. Según el Ministerio de Salud del Perú, las principales causas de mortalidad neonatal son prematuridad, infecciones, asfixia y malformaciones congénitas.

Actualmente, la neonatología se considera una rama de la medicina altamente especializada, la cual contribuye al logro de los objetivos de desarrollo sostenible. Para el año 2030, esta iniciativa de la Organización de las Naciones Unidas tiene como objetivo reducir la mortalidad infantil, brindando atención especializada en situaciones críticas y promoviendo el bienestar del neonato. Siendo estos desafíos complejos con enfoques multidisciplinarios e innovadores, se requiere la participación activa de grupos de trabajo colaborativo y redes de investigación entre especialidades afines. El objetivo de esta carta es destacar lo fundamental de estas dos prácticas para el desarrollo futuro en beneficio de la atención del recién nacido.

The use of inhaled nitric oxide is part of the therapeutic strategies in the newborn with severe pulmonary pathology associated with pulmonary hypertension. The nitric oxide allows an improvement in oxigenation by promoting pulmonary vasodilation. As inclusion criteria we have pulmonary hypertension diagnosed by echocardiography and hypoxemic respiratory failure. Patients with left ventricular dysfunction, intracraneal hemorrhage, chromosomopathies or malformations incompatible with life are not eligible. The objective of our publication is to describe our protocol for the use of inhaled nitric oxide in pulmonary hypertension of the newborn at “Instituto Nacional del Niño-San Borja”, in Lima Perú.
