The cleft lip and palate is the most common congenital malformation between head and neck anomalies. It has an impact on the quality of life of the patient and his family, therefore a multidisciplinary and timely management of its sequels is fundamental for his development. The purpose of this clinical case was to show the integral dental management of a patient diagnosed with bilateral cleft lip and palate, female, 14 years old, who went with her mother to the Institute of Craniofacial Malformations of the University of Chile because they wanted a solution for her altered facial aesthetics and correction of dental malposition. The cleft lip and palate is a congenital craniofacial malformation that develops from embryological defects during the formation of the face. The anomalies occur mainly in the upper lip, premaxilla, palate and nasal floor. Here we introduce a case report of a patient who was admitted at 7 years old diagnosed with bilateral cleft lip and palate treated in the Craniofacial Malformations Unit of the Faculty of Dentistry of the University of Chile. The patient presented a Class III skeletal relationship, lack of development of the facial middle third, protruding position of the premaxilla, a bilateral Class III molar malocclusion with malposition and dental crowding. The cleft lip and palate is the most common congenital malformation among craniofacial anomalies and causes a series of functional, physiological, aesthetic and social alterations. One commonly characteristic associated with these patients is a severe maxillary hypoplasia as a result of the great resistance opposed by scar lip and palate tissue from a primary surgery. Traditionally, the suggested treatment has been conventional orthognathic surgery at the end of skeletal maturity. However, this approach has revealed certain limitations. La fisura labiopalatina es una malformación congénita craneofacial que se desarrolla a partir de defectos embriológicos durante la formación de la cara. Las anomalías se presentan fundamentalmente en el labio superior, la premaxila, el paladar y en el piso nasal. A continuación se presenta un caso clínico de una paciente que ingresó a los 7 años de edad diagnosticada con fisura de labio y paladar bilateral tratada en la Unidad de Malformaciones Craneofaciales de la Facultad de Odontología de la Universidad de Chile. La fisura labio-palatina es la malformación congénita más común entre las anomalías craneofaciales y causa una serie de alteraciones a nivel funcional, fisiológico, estético y social. Una de las características comúnmente asociada a estos pacientes, es la hipoplasia maxilar severa producto de la gran resistencia opuesta por los tejidos cicatrizales de labio y paladar, como consecuencia de la cirugía primaria. Tradicionalmente, el tratamiento ha sido la cirugía ortognática convencional al término del crecimiento, sin embargo, este enfoque ha demostrado tener ciertas limitaciones.
