Gerbode Defect (GD) is a rare congenital heart disease that mainly affects the upper portion of the membranous septum, generating a shunt between the left ventricle and the right atrium. Even though most cases are congenital, it has also been reported acquired cases due to cardiac surgery, infective endocarditis, acute ischemic heart disease, and invasive percutaneous procedures. The diagnostic workup includes the clinical evaluation and the echocardiographic study. Here, we present the case of a 43-year-old adult patient with an incidental finding of a congenital GD in the context of acute appendicitis. Imaging plays a role in the diagnostic workup of congenital diseases; in this case, it allowed us to identify more details and the decision- making for our patient.

Los síndromes coronarios agudos sin elevación del segmento ST (SCASEST) varían en su presentación: desde cuadros autolimitados, hasta aparatosos y desenlace fatal. Las guías actuales enfatizan la necesidad de estratificar el SCASEST en escalas de riesgo: muy alto riesgo (MAR), alto riesgo (AR) y bajo riesgo (BR) con la finalidad de priorizar o no la estrategia invasiva (cateterismo y angioplastia coronaria), esto debido a aparente mal pronóstico en SCASEST-MAR. OBJETIVO: Identificar las características epidemiológicas, clínicas y quirúrgicas de pacientes sometidos a relaparotomía en el Hospital Base Almanzor Aguinaga Asenjo 2014-2016. METODOLOGÍA: estudio observacional, descriptivo y transversal. La población estuvo conformada por pacientes mayores de 14 años cuya reintervención quirúrgica fue realizada dentro de los 60 días postoperatorios. Se trabajó con la población total o universo de 123 pacientes, realizándose revisión sistemática de expedientes clínicos. RESULTADOS: La tasa de relaparotomía global fue de 5.82%, 50.42% fueron mayores de 60 años, el sexo femenino se presentó en 51.2%. 61.8% presentó alguna comorbilidad, el 61.8% fueron cirugías de emergencias. Objective: To describe the clinical and epidemiological characteristics, evolution and treatment of patients with pulmonary arterial hypertension.Materials and methods: A descriptive study that included 47 patients with a primary diagnosis of pulmonary arterial hypertension. Results: The average age of the patients was 48 years and 91.49 % were females. The average follow-up was 39 months.The main types of pulmonary arterial hypertension were the one associated with congenital heart diseases (48.93 %), the one associated with connective tissue diseases (21.28 %) and idiopathic hypertension (17.02 %). Monotherapy (mainly with sildenafil) and sequential combination therapy (sildenafil plus bosentan) were used in 78.72 % and 21.28 % of the patients,respectively.
