Objective: To evaluate the association between delayed surgical treatment and complicated acute appendicitis in patients treated at the Loreto Regional Hospital during 2024. Methodology: An observational, analytical, and cross-sectional study was conducted with 222 patients diagnosed with acute appendicitis, in whom clinical and surgical variables were analyzed. Results: Patients with complicated appendicitis presented longer delays in all stages of the care process. The time from symptom onset to hospital admission was significantly longer in this group, with more visits after 24 hours. Likewise, surgical waiting time and the frequency of delays ≥24 hours were higher in complicated cases.
